Feedback subjects and forms

Choose clear subjects, edit questions and publish without changing your QR links.

What is a feedback subject?

A subject groups related responses. Products and batches suit product reviews. Services and branches suit customer experience. Employee, event, team, room and table subjects identify a more specific experience. A suggestion box collects open ideas.

A branch record is an organizational location. A branch-type feedback subject is what people review. Associate the subject with the correct branch and department when you need scoped staff access.

Create and organize a subject

  1. Open Feedback targets in the main workspace navigation, or choose Create Feedback in your personal workspace.
  2. Choose the type and a name customers will recognize.
  3. Expand optional organization fields to assign a branch, department or parent subject. These choices also affect scoped access.
  4. Review and create. Lipback publishes one starter form for the subject.

Set up the form

Open Forms and select Set up feedback form. Select or create the subject, configure the name and customer messages, then review before publishing. This edits the existing starter form rather than creating a duplicate.

Edit questions and templates

Use Edit form to change question labels, answer formats, order and required flags. A starter template replaces the draft questions; review before using it. Supported formats include emoji, stars, satisfaction, recommendation, effort, yes/no, choices, scale and text.

Use up/down buttons or drag questions to reorder them. Conditional questions must refer to an earlier answer. Test both matching and nonmatching answers. Require only the information you actually need.

Drafts, publication and archiving

Save draft does not replace the customer-facing version. Save & publish creates the version used by the existing subject link and QR codes. Previous responses retain their original version.

Archiving makes the subject unavailable for new feedback and new QR/form creation. Historical records remain. Reactivation can be restricted by the active-subject allowance of your plan.

Private image attachments (paid plans)

The optional private image field is available on feedback forms owned by an active Starter, Business, Growth or Enterprise workspace. Free and personal workspaces do not accept feedback attachments. Visitor or Conversation trials on Free do not unlock this feature.

Customers can attach JPEG, PNG or WebP images up to 5 MB, with a maximum of three per response. Images are validated and re-encoded, remain private, and can be downloaded only by authorized staff. Do not upload clinical records, identity documents or other sensitive material.

The upload service checks the current subscription again, including after a downgrade. Previously uploaded images are retained under the applicable retention policy; a downgrade does not delete them.
